Understanding Axial Length: What the Physical Size of Your Eyeball Means for Your Vision

When you go to the eye doctor, you usually walk away with a prescription number like -2.00 or -5.00. Most people know this determines how blurry their distance vision is. However, what your doctor is actually measuring implicitly is the physical size of your eye—specifically, its axial length.

Axial length is the distance from the very front of your eye (the cornea) to the very back (the retina). In a perfectly clear-sighted eye, this distance is usually between 23.0 mm and 23.5 mm.

When someone develops nearsightedness (myopia), it means their eyeball has grown too long from front to back. For every single extra millimetre of growth, the eye gains roughly -2.50 to -3.00 dioptres of prescription.

Here is how different physical eyeball lengths break down and what they mean for your health.

The Four Categories of Eye Length

  • Normal / Baseline (23.0 mm – 23.5 mm): Light enters the eye and focuses perfectly on the retina. No distance vision correction is needed.
  • Mild Myopia (24.0 mm – 25.0 mm): The eyeball has slightly elongated. This usually correlates with a light prescription up to -3.00 dioptres.
  • Moderate Myopia (25.0 mm – 26.0 mm): Elongation is moderate, generally resulting in a prescription between -3.00 and -6.00 dioptres.

Why We Measure Length, Not Just Prescriptions

Measuring axial length using advanced optical scans has become the gold standard in modern eye care, especially for children.

Because a child’s eyes grow as they grow, measuring the physical millimetres of the eye allows us to catch rapid elongation before it shows up dramatically on a standard vision chart. If we see the axial length creeping closer to that 26 mm mark, we can proactively intervene with modern myopia control therapies—such as specialized overnight lenses or targeted eye drops—to slow down physical eyeball growth.
